Describe what is visible
Write crack, chip, stain, scratch, loose hinge, worn finish, or another observable condition. Avoid assigning fault or stating a cause you cannot verify.
Show where the mark is
Name the room, area, and nearby landmark, then place a marker on the exact point in the photo. Numbered markers help when one image contains several marks.
Add useful scale and lighting
Take one photo that shows the area and another close enough to see the mark. If size matters, include an ordinary familiar object without covering it.
Keep later edits clear
If a note needs correction, update the wording while keeping the correct photo and its details. Do not replace a clear observation with a stronger claim.
